After years in traditional primary care settings, our founder recognized a core issue: the healthcare system often fails to meet its fundamental purpose.
Patients often feel rushed. Continuity of care is fragmented. Prevention is too often delayed.
In response to these challenges, Somaform Health was created to restore what has been lost, relationship-centered care. We envision a model in which patients are seen, heard, and cared for as individuals, rather than being reduced to their conditions.
